Parental Consent for Sports Fixtures

In the course of this academic year your child may be selected to play in sports fixtures off site. In order for them to participate it is now necessary for you to give your consent. Please complete the online form below as soon as possible. Students cannot attend off site sporting fixtures without a completed consent form. If you have more than one child at this school then please complete separate forms for each child.

Teachers leading a fixture will always have with them contact numbers and medical information. We will be using the school database for this purpose but request that any changes are reported to the Head of Physical Education so that the teachers have the most up-to-date information.

We very much appreciate and value the support parents/carers give us with fixtures.

I agree to inform the Head of Physical Education of any changes in my son’s/daughter’s medical or other conditions or any other relevant circumstances.*

I agree to my son/daughter receiving any and all emergency medical treatment, including anaesthetic and/or blood transfusion, as may be considered necessary by the medical authorities in attendance should the need arise.*
